1. Acceptance
By creating an account, booking an appointment, or otherwise using DocConnect, you agree to these Terms & Conditions and our Privacy Policy. If you do not agree, please do not use the service.
2. Who we are
DocConnect is a practice management and telemedicine platform operated by individual licensed physicians in the Philippines. The specific data controller and service provider for this instance is the physician whose practice you are accessing.
3. Services offered
- Online appointment booking with a registered physician
- Secure exchange of health information and messages
- Issuance of electronic prescriptions and medical certificates
- Optional teleconsultation (where enabled and legally permitted)
- Online and on-site payment collection
4. Medical services disclaimer
DocConnect is a software platform; it is not a medical provider. The physician using this system is solely responsible for the medical advice, diagnoses, prescriptions, and treatment decisions rendered to you. The platform does not replace emergency medical care — in life- threatening situations, call your local emergency number or proceed directly to the nearest hospital.
5. User responsibilities
- Provide truthful and up-to-date personal and medical information
- Keep your login credentials (password, authenticator, OTP codes) private
- Honor booked appointments and pay any applicable fees
- Use the service only for lawful personal healthcare purposes
- Do not impersonate another patient or attempt to access another user's records
6. Payments, cancellations & refunds
Consultation fees are set by the treating physician and displayed before you confirm a booking. Cash collected on-site is evidenced by the physician's official receipt. Online payments are processed by PayMongo; DocConnect does not store card or e-wallet credentials. Refund and cancellation policies are set by the treating physician and may be communicated at booking. Appointments may be cancelled up to the time of the scheduled visit; late cancellations may be subject to a fee at the physician's discretion.
7. Senior citizen & PWD discounts
Where applicable under Republic Act 9994 (Expanded Senior Citizens Act) and Republic Act 10754 (Magna Carta for Persons with Disabilities), qualified patients are entitled to a twenty percent (20%) discount and VAT exemption on medical consultations. Present your senior-citizen or PWD identification card when booking or upon arrival at the clinic to claim the discount.
8. Data privacy
Your personal and sensitive personal information is processed in accordance with Republic Act 10173 (Data Privacy Act of 2012). Full details of how we collect, use, store, and share data are in our Privacy Policy, which forms part of these Terms.
9. Electronic prescriptions & certificates
Prescriptions and medical certificates issued through DocConnect bear the physician's PRC license number and electronic signature and may be printed for presentation at pharmacies, employers, or other authorized recipients. They carry the same legal weight as paper documents issued by the same physician.
10. Limitation of liability
To the maximum extent permitted by law, DocConnect and its operator are not liable for indirect, incidental, or consequential damages arising out of your use of the service. Nothing in these Terms excludes liability for death, personal injury, gross negligence, or willful misconduct.
11. Termination
Either party may terminate this agreement at any time. Upon termination, your records will be retained for the period required by Philippine law (currently fifteen years for adult medical records) after which they may be securely disposed of in accordance with our retention policy.
12. Changes to these terms
We may update these Terms from time to time. The effective date at the top of this page reflects the most recent version. We will notify active users of any material change before it takes effect.
13. Governing law
These Terms are governed by the laws of the Republic of the Philippines. Any dispute that cannot be resolved amicably shall be submitted to the exclusive jurisdiction of the courts of the city where the treating physician's clinic is registered.
